Wet Primed Extracorporeal Circuit for Up to 30 Days.

Background : As reduction of extracorporeal membrane oxygenation (ECMO) initiation time in urgent patients drastically reduces morbidity and mortality, it is important to aseptically assemble, prime and de-air the extracorporeal circuit (ECC) as quickly as possible. Availability of pre-primed ECC has critical importance in saving time for urgent situations especially in centers without 24-hour-ready ECMO team.

Methods : One hundred and thirty-two patients (age>18 years) were treated with ECMO from January 2011 to May 2016. Pre-primed ECCs have been used in recent 21 patients. We have reviewed electronic medical records of these 21 patients(Group A) to compare with patients treated with non-pre-primed ECC(Group B).

Results : Mean duration from ECC pre-priming to initiation of ECMO was 10.0 ± 8.6 days (max, 30 days). For E-CPR cases (n=27), CPR time was 47.7 ± 22.8 minutes in group A (n=3) compared to 54.0 ± 33.5 minutes in group B (n=24)(p=0.349). Time from the decision to the initiation of ECMO was 23.7 ± 18.5 minutes in group A (n=3) compared to 43.5 ± 17.3 minutes in group B (n=24)(p=0.110). In group A, eleven patients (52.4%) were weaned from ECMO, and duration of ECMO run was 6.2 ± 10.3 days. There was no proven blood-stream infection during ECMO. No mechanical problems from keeping the pre-primed ECMO flow on while standing-by was found.

Conclusion : Pre-assembling and priming the ECC for urgent ECMO initiation is effective and time-saving. Sterility was preserved up to 30 days in our experience. Further study with larger number of patients is needed.
