Image-guided Thoracoscopic Pulmonary Metastasectomy Using Near Infrared Thoracoscope

Background : The aim of this study is to develop the protocol of localization for image-guided surgery of solitary pulmonary nodule with indocyanine green (ICG) and fluorescent video surgical system.

Methods : The patient was a 48-year old male who had two solitary pulmonary nodules at left lower lobe, which were diagnosed to pulmonary metastasis from colon cancer. In this case, radiologic interventionist performed preoperative CT-guided hook-wire localization with lipiodol and ICG mixture before 1-hour from the operation. We performed uniportal VATS pulmonary metastasectomy with real-time visualization of the ICG signal confined only around the target lung lesion. In this video, we also confirmed the pleural location with hook-wire and the stapling resection margin with C-arm fluoroscopy.

Results : The lipiodol was useful to prevent the spread of less viscous fluorescence material (ICG) after localization. We also performed hook-wire insertion for validating the correct pleural location of the lung lesion. Fluorescent video surgical system could see the ICG fluorescent signal not spread during operation. We also could see the inadequate resection margin from the target lesion when stapling.

Conclusion : Our protocol with ICG and fluorescent video surgical system might be a future fluorescence image-guided resection of lung nodule with real-time visualization of the pleural location and adequate resection margin.
