Surgical Treatment of Atrial Septal Occluder-related Complications; Mitral Valve Perforation and Aggravated Mitral Regurgitation

Background : Atrial septal defect(ASD) transcatheter occlusion techniques have become a reliable alternative to surgical procedures. Possible complications may derive from unfavorable anatomy of the defect and over-dimensioning of the device.

Methods : We describe the case of anterior mitral leaflet perforation caused by an atrial septal occluder and aggravation of mitral regurgitation(MR) due to the procedure-induced femoral arteriovenous fistula.

Results : Device removal, ASD closure, and mitral repair were successfully performed utilizing minimally invasive cardiac surgery(MICS).

Conclusion : ASD can successfully be closed with MICS and the device extracted after failed transcatheter closure with excellent results. We suppose worthy of consideration in implementing MICS approach from the outset rather than taking risk by performing device closure in the event of large defect size or anatomically nonoptimalized
