A Succefully Treated Case using Tractotomy Technique for a Penetrating Lung Injury in One-lung Patient

Background : We usually manage penetrating lung injury patient with simple suture ligation or anatomical lung resection. But sometimes it may induce complications. Simple suture ligation would cause intrathoracic hematoma or air embolism. If large volume anatomical resection is done, residual lung function would be limited. Tractotomy can avoid these complications.

Methods : 61 year old male patient visited our emergency department due to hemoptysis and dyspnea after chest injury at work. He had previous medical history of left pneumonectomy due to intractable pulmonary tuberoculosis 24 years ago. Chest X-ray and chest CT scan was done and diagnosed as penetrating lung injury with metal fragment in lung. We did perform emergent tractotomy operation with intermittent lung down of right lung.

Results : Foreign body removal and bleeding control were successfully done, and the patient was discharged after 5 days without any complications.

Conclusion : Tractotomy is very useful and safe especially in patients who has limited lung function because it helps to preserve lung function and decrease operative time. It can prevent intrathoracic hematoma and air embolism by direct control of bleeding and air leak lesions. It can be very helpful in penetrating lung injury.
