The Potential Role of Hemoperfusion with Polymyxin B Adorption Filter for Immunomodulatory Effect in Canine ECMO Model

Background : Despite of significant advancements of extracorporeal membrane oxygenation (ECMO) system, we are unable to completely control the subsequent inflammatory and immune-paralysis reactions to the use of ECMO. Hemoperfusion with Polymyxin B adorption filter is considered to potentially improve the clinical outcomes with anti-inflammatory effect. However little is known about the effect of immune-regulation by this device. The aim of our study is to identify the potential role of this device for immuno-regulation in canine ECMO model.

Methods : Twelves canines (median body weight: 9.5kg) were randomly divided into 4 groups: control group, ECMO group and ECMO+filter group. Venoarterial-ECMO model was made in 6 canines with cervical cannulation under general anesthesia. Hemoperfusion with Polymyxin B filter was started at hour 6 after initiation of ECMO in ECMO+filter group. Blood samples were taken from the animals at 0, 3, 6, 9 and 12 hours during experiment. Cytokine (IL-6 and IL-10) was measured by real-time PCR analysis and flow cytometry analyses were performed for identification of immune cell distribution.

Results : Real-time PCR analysis demonstrated that IL-6 and IL-10 was reduced in ECMO+filter group. However, a progressive increase was detected in ECMO group (p<0.05). Flow cytometry analyses revealed that immune cell count was significantly reduced in control group. On the other hand, the decrement of immune cells was alleviated in filter group compared to the control group.

Conclusion : We suggest that an immunomodulatory effect might exist during hemoperfusion with Polymyxin B Filter in canine ECMO model. However further investigation will be needed.
