Factors Associated with Lymph Node Metastasis in T1 Esophageal Cancer

Background : In esophageal cancer, LN metastasis is an important prognostic factor. Lymphadenectomy is important, but in some cases, we could not perform aggressive lymphadenectomy. So, we intend to evaluate factors associated lymph node metastasis in surgically resected T1 esophageal cancer.
Methods : From 2004 to 2013, total 87 patients who diagnosed as T1 esophageal cancer were performed esophageal resection and reconstruction in Seoul National University Bundang Hospital. Among them, patients received preoperative neoadjuvant therapy and had other malignancy were excluded and we reviewed 52 patients, retrospectively.
Results : The median age was 65 years (40 ~ 75 years). Male patients were 50 (96.2%), and female were 2(3.8%). According to pathologic report, squamous cell carcinoma were found at 50 (50/52, 96%), and T1a stage were 23 (23/52, 44.2%), T1b stage were 29 (29/52, 55.8%). The median number of resected lymph node were 30 (9 ~ 87). Lymph node metastasis were found at 13 (13/52, 25%) patient, and the 5 year survival rate in LN (+) and LN (-) group were 74% and 93% (p=0.025). Angiolymphatic invasion was independently associated with lymph node metastasis (p=0.002), but T stage, tumor size, and grade were not associated with the risk of nodal disease.
Conclusion : In our study, LN metastasis was significantly associated with survival rate. And the angiolymphatic invasion was an independent factor of lymph node metastasis in T1 esophageal cancer.
