Reconstruction of Aorto-mitral Continuity with Bovine Pericardium; Surgical Outcome for 8 Years

Background : We report our mid-term surgical results in patitents with damaged aorto-mitral intervalvular fibrosa (IVF) by approaching the aortic and mitral valves through Manouguian incision between the aortic root and dome of the left atrium.

Methods : The clinical profiles were examined retrospectively by chart review. Reconstruction of the aorto-mitral intervalvular fibrosa (IVF) with bovine pericardium through the Manouguian incision was done in 9 adult patients between Aug 2007 and Aug 2015 because of infective endocarditis with annular abscess in 8 and prosthetic valve failure with extensive calcification in 1. Six patients had had previous operation.

Results : The aortic and mitral valves were replaced in all patients, Bentall’s procedure in 2. There was no death in perioperative period. Patients were followed up from 3 to 93 months, with a mean of 49 months. There was 1 late death by rectal cancer. There was no late patch dehiscence.

Conclusion : In conclusion, reconstruction of aorto-mitral intervalvular fibrosa (IVF) with bovine pericardium during aortic and mitral valve replacement in patients with infective endocarditis or heavy calcification is extremely effective.
