Classic Peripheral Signs of Subacute Bacterial Endocarditis

Background : The peripheral signs of endocarditis is well known, but less common after advance of diagnostic modalities
Methods : A 50-year-old female with visual disturbance presented to the cardiothoracic surgery via ophthalmologist referral due to cardiac murmur. Fundoscopy revealed the Roth’s spot on her both eyes. Physical examination showed peripheral signs of infective endocarditis include Osler’s node, Janeway lesions and splinter hemorrhage.
Results : She diagnosed as subacute infective endocarditis with severe aortic regurgitation which meets the all Duke criteria except predisposing heart disease or intravenous user. She underwent aortic valve replacement and treated with intravenous antibiotics for 6 weeks. The patient has made a remarkable recovery and discharge without any symptoms.
Conclusion : We report a case of subacute endocarditis with four classic peripheral signs who meets 2 major and 4 minor Duke criteria.
