Background : Acute ascending aortic dissections should be treated as a surgical emergency since their high risk of life-threatening complications. However, deciding surgery after occurrence of cardiac arrest is very difficult.

Methods : We are about to introduce two successful surgical cases of these kinds of patients

Results : One case is a 38-years old patient who transferred to our hospital after diagnosed with Aortic dissection. The patient is arrested while waiting a surgery and the cardiopulmonary bypass was initiated after 42 minutes of arrest. After surgery, there was no patient’s immediate postop complication except the fact that patient found out to be brain death which led to donor his organs. Second case is a 90-years old man who was return of spontaneous circulation after ten minutes of resuscitation moved onto operation room. The patient undergone surgery and discharged with no postoperative complications. The patient survived after discharge and followed up in out-patient clinic for six months.

Conclusion : Surgical repair of acute type A aortic dissection is challenging, but can be performed with immediate cardiopulmonary resuscitation.
