Impact of Lifestyle Disease on Postoperative Complications and Survival in Elderly Patients with Stage I NSCLC

Background : The influence of lifestyle disease on postoperative complications and survival for patients with NSCLC are unclear. The aim of this study was to evaluate whether lifestyle diseases are significant risk factors of perioperative and long-term surgical outcome in elderly patients with stage Ⅰ NSCLC.

Methods : Between December 1995 and November 2013, 110 patients, aged 65 years or older, who underwent surgical resection for stage Ⅰ NSCLC at Dong-A University hospital were retrospectively studied. We assessed each lifestyle diseases (such as diabetes mellitus, hypertension, chronic obstructive pulmonary disease, cerebrovascular accident and coronary artery disease) and lifestyle diseases status of the patients as risk factors for postoperative complications and survival.

Results : The patient’s mean age was 71 years (range, 65~82 years). 46 patients possessed HTN as the most common lifestyle disease with 41.8%, which is followed by 23 DM patients with 20.9%. Hospital mortality was 0.9%(1 patient). Three-year and five-year survival was 78% and 64%, respectively. Postoperative complications developed in 35(31.8%) patients, including 7(6.4%) with prolonged air leak, 6(5.5%) with atrial fibrillation, 5(4.5%) with delirium and atelectasis, 3(2.7%) with AKI and pneumonia and so on. Univariate and multivariate analysis showed that the lifestyle disease status was only independent risk factors for postoperative complications. Moreover, in survival rate, univariate analysis showed that age, smoking, BMI, resection type, stage were significantly associated with an impaired survival. Multivariate analysis showed that resection type [OR:2.20 (1.08~4.49) p=0.030] and stage [OR:1.89 (1.02~3.49) p=0.043] were associated with an impaired survival.

Conclusion : The present study demonstrated that the lifestyle disease status is a significant prognostic factor of postoperative complications, but of survival rates in elderly stage I NSCLC patients. Therefore, it is assumed that closer observation and aggressive management after surgery would be necessary for patients with lifestyle diseases.
