Clinical Outcomes of Aortic Root Reimplantation and Bentall Procedure: A Propensity Score Matching Analysis

Background : The aim of the study was to evaluate the clinical outcomes of aortic root replacement (ARR); valve-sparing root replacement (VSR) and the Bentall procedure.

Methods : We retrospectively reviewed 265 patients who underwent ARR between 1995 and 2013 at Samsung Medical Center. Patients were divided into two groups: Bentall (n=166) and VSR (n=99). The mean follow-up duration was 68.5±54.1months.

Results : Patients in the Bentall group were older than those in the VSR group (Bentall, 50±14years; VSR, 37±16years, p<0.001). Incidence of Marfan syndrome (Bentall 46, 27.7%; VSR 63, 63.6%, p<0.001) and AR≥4 (Bentall 93, 56.0%; VSR 15, 15.2%, p<0.001) was different between the groups. There were no early deaths in the VSR group and three in the Bentall group (p=0.295). Early morbidities were not different between two groups. Despite the higher reoperation rate in the VSR group than in the Bentall group, overall mortality and major adverse valve-related events (MAVRE) did not differ between the two groups. Propensity score adjusted analysis revealed that no difference between the two groups to experience overall mortality and MAVRE. We performed a propensity score-matching analysis comparing the two groups (n=214). Matched analysis also revealed that no difference between the two groups to experience overall mortality and MAVRE.

Conclusion : Surgical technique for ARR did not appear to affect survival and valve-related events. VSR can be a viable option in patients who do not want to have life-long anticoagulation.
