The Clinical Implication of Paired-like Homeodomain Transcription factor 2 (PITX2) Expression in the Adenocarcinoma of the Lung

Background : Lung cancer has higher mortality rate and also has higher frequency of recurrence and metastasis, then other cancer. Therefore to find prognostic or therapeutic marker is necessary.
PITX2, originally identified as an involver in multiple organogenesis during embryonic period, it has been reported that PITX2 may involve in oncogenesis and cancer progression, especially in the thyroid, breast, and prostatic cancer. The purpose of this study was to evaluate PITX2 expression and its clinical implication in the adenocarcinoma of the lung with analysis for correlation with clinicopathologic prognostic factors and survivals


Methods : Immunohistochemical analysis was made on the paraffin-embedded tissues of 56 cases of lung adenocarcinoma which could provide the data for survival analysis. The immune reaction with more than 10% of tumor cells was interpretated as positive. Positive reaction was classified as 'high-expression' and 'low-expression'. Kaplan-Meier survival analysis was performed

Results : 5 year survival rate of these patients with lung adenocarcinoma was 50.00% (28/56). Forty-three of 56 cases (76.8%) showed cytoplasmic expression, among which 23 cases showed high-expression (41.1%) and 20 cases (35.7%) showed low-expression. Negative reaction was in 13 cases (23.2%). In stage I, 10/37 cases (27.0%), showed negative reaction, 16/37(43.2%) showed high-expression and 11/37(29.7%) showed low-expression. In stage II, high- and low expression was in 3/7(42.9%) in each. In stage III, high-expression showed 3/12(25%), low-expression 7/12(58.3%). There were no significant relationship of the expression with age, gender, and stage However, Kaplan-Meier survival analysis showed significant higher survival in high-expression group than low-or-negative expression groups(p=0.009), especially in stage I showed same significant higher survival in high-expression group(p= 0.009)

Conclusion : These results may suggest that 'high-expression of PITX2' can be the prognostic factor toward longer survival and PITX2 immunohistochemical staining could be useful tool for detecting favorable prognostic factor
