The Surgical Results of Monocusp Implantation with TAP in TOF Repair.

Background : Transannular patch angioplasty (TAP) is sometimes inevitable during Tetralogy of Fallot (TOF) repair. This study investigates the surgical efficacy of monocusp implantation with TAP during TOF total repair.

Methods : Thirty six patients (mean age, 12.5 months, mean body weight; 8.7 kg, M:F=18:18) who performed the TAP during TOF total repair between 2000 and 2009 were retrospectively analyzed and classified into two groups: group I, monocusp implantation group (n = 25); group II, no monocusp implantation group (n = 11).

Results : There was no operation related mortality in this study. There was no statistical difference in immediate postoperative pulmonary valve velocity between two groups (group I: 2.0 ± 0.5, n=25, group II: 1.5 ± 1.0, n=11, P = 0.189). There were 3 patients who developed the pulmonary valve velocity more than 3.5 m/s in group I (mean 7.3 years) during the follow-up. Although there was no difference of more than grade 3 degrees pulmonary regurgitation (PR) incidence between two groups (68.0% in group I, 72.7% in group II, P = 0.777) during the follow-up, the interval to the more than grade 3 PR was prolonged in group I compared to group II (group I: n=17, 6.5 ± 3.4 years, group II; n=8, 3.9 ± 2.2 years)(P=0.037).

Conclusion : Monocusp implantation with TAP in TOF repair is feasible and durable procedure.
