Non-surgically or Surgically Treated Mild Rheumatic Mitral Dysfunction in Patients Undergoing Aortic Valve Replacement

Background : Considering the progression of rheumatic pathology, it is sometimes controversial whether to surgically manage the mild rheumatic mitral dysfunction for patients undergoing aortic valve replacement.

Methods : From January 1989 to June 2012, a total of 2731 patients underwent aortic valve replacement. Among these, 88 patients (mean age, 56.4 ± 12.0 years; 43 females) with mild rheumatic mitral dysfunction, defined as mitral regurgitation of proximal isovelocity surface area (PISA) < 4mm or mitral stenosis of mitral valve area ≥ 1.5 cm2, were screened. Of whom 58 patients had no MV surgery and 30 patients had MV surgery. Freedom from valve-related events was compared between these two groups of patients.

Results : There was one in-hospital-mortality and no significant differences in early complication rates between the two groups. During the median follow-up period of 103.1 (interquartile range, 65.9 - 133.1 months) months, there were 6 (6.8%) late deaths and 21 (23.9%) valve-related events. Freedom from valve-related event at 5 years was 84.1 ± 4.9% versus 83.3 ± 6.8% and 10 years was 75.2 ± 6.5% versus 79.4 ± 7.5% (P = .57).

Conclusion : Conservatively treated patients with mild rheumatic mitral dysfunction showed acceptable clinical outcomes.
