Single Port Three-dimensional Thoracoscopic Sleeve Right Lower Lobectomy with Mediastinal Lymph Node Dissection

목적 : This video demonstrates the single port thoracoscopic sleeve right lower lobectomy and lymph node dissection under three-dimensional (3D) video system.
방법 : A 41-year-old male patient, with a history of recurrent pneumonia due to bronchial glanular cell tumor at right lower lobar bronchus, was referred to our department for surgical treatment after multiple cryotherapy to endobronchal lesion. Sleeve right lower lobectomy was performed through A 3.5-cm incision single port VATS under the 3D video system.
결과 : Although full mobilization of interlobar pulmonary vessels and lobar bronchus, resection margin was not adequate for stapling. Under the assist of intraoperative bronchoscope, we performed sleeve resection and end to end anastomosis of right middle lobar bronchus and bronchus intermedius via single port 3D VATS with clear margin. The chest drain was removed on the 6th postoperative day. The patients was discharged without complication on the 9th postoperative day.
결론 : A sleeve lobectomy can be performed safely via single port VATS. Three-dimensional video system is helpful and fascillitate the complex procedures, such as suturing, bronchotomy, anastomosis, and excision of mediastinal lymph node during single port VATS.
