Early Results of Modified Aortic-valve Sparing Partial Root Replacement in Acute Type a Aortic Dissection with an Intimal Tear in the Aortic Root

Background : The aim of this study was to evaluate operative and early results of a modified aortic-valve sparing partial root replacement (AVSPRR) in acute type A aortic dissection with an intimal tear in the aortic root.

Methods : Between March 2010 and August 2014, five patients (mean age 50.4 ± 14.5 years) underwent a modified AVSPRR. This surgical procedure consisted of selective replacement of the coronary sinus containing an intimal tear and external wrapping of the non-coronary and torn coronary sinuses with a rectangular vascular graft followed by reimplantion of coronary button.

Results : Aortic valves were successfully preserved in all patients. Post-operative echocardiography in the all patients revealed that mean aortic root size was 37.4 ± 2.5 mm, with mild aortic insufficiency in one subject and trivial in the other four subjects. There was one in-hospital mortality. During the follow-up period (mean 6.8 ± 4.1 months), four survivors remained asymptomatic, and there were no valve-related events or reoperations on the proximal aorta.

Conclusion : In acute type A aortic dissection with an intimal tear in the aortic root, our modified AVSPRR led to a feasible short-term outcome, and may be considered an option in surgical treatment of selected patients.
