Robotic Esophagectomy by Dual Console Four-arm Technique

Background : We report a case of robotic esophagectomy and lymph node dissection by dual console four-arm technique in esophageal cancer.

Methods : The tumor was located at 36cm from upper incisor and clinical stage was T1bN0M0. The patient was positioned in prone. Four ports for robotic arms and one port for assistant were placed. Number 1 and 2 robotic arms were controlled by operator in main console. Number 3 robotic arm was controlled by assistant in 2nd console. The main console used robotic scissors and Maryland bipolar forceps. The 2nd console used Cadiere forceps or robotic suction/irrigator.

Results : The operation was completed successfully without intraoperative event. Total operation time was 190 minutes for esophagectomy. While the main operator was performing procedure, the assistant could offer effective retraction and clear field for dissection without interruption.

Conclusion : Dual console four-arm technique enabled maximal utilization of robotic technique. Furthermore this technique facilitated easy and safe operation, and offered an assistant to experience robotic surgical technique by close communication and immediate feedback from the main console.
