Aortic Pseudoaneurysm in Seronegative Undifferentiated Spondyloarthropathies

Background : Aortitis with undifferentiated spondyloarthropathies is very rare and difficult to diagnose. We would describe pseudoaneurysm of ascending aorta with axial spondyloarthritis.
Methods : A 26-year-old female, who was treated for pulmonary tuberculosis and sacroiliac joint abscess 3years ago, was admitted our hospital complaining of hoarseness. At echocardiogram and chest computer tomography(CT) scans revealed large mass, 110mm in maximum diameter, with thrombus internally in anterior mediastinum arising from ascending thoracic aorta and large mass compress the ascending aorta and main pulmonary trunk.
Results : At surgery, there was severe adhesion and inflammation around pseudoaneurym, intraluminal fresh and old thrombus and fistula about 5mm diameter connecting aorta. The resection of pseudoaneurysm, patch closure of ascending aorta and Interposition of omental flap were performed successfully under an axillary arterial cannulated CPB.
The intraoperative ordinary culture, acid-fast bacilli(AFB) stain, culture and TB-PCR from biopsy tissue, and Antinuclear antibody(ANA), antineutrophil cytoplasmic antibody(ANCA), rheumatoid factor, human leukocyte antigen B-27 (HLA B-27) were negative results. Pathologic examination of the pseudoanurysmal wall demonstrated fibrosis of the connective tissue with numerous lympocytic infiltration, and no caseous necrosis.
She has been well with the immunotherapy in the last 24 months.
Conclusion : We report the rare case of pseudoaneurysm of ascending aorta with seronegative undifferentiated spondyloarthropathies.
