Which is better: Preservation or Division of the Inferior Pulmonary Ligament during upper Lobectomy of the Lung?

Background : It is uncertain whether the division of the inferior pulmonary ligament is necessary in non-small cell lung cancer (NSCLC) patients who undergo upper lobectomy. The aim of this study is to investigate the relationship between the division of the inferior pulmonary ligament and postoperative complications.

Methods : Seventy-two NSCLC patients underwent video-assisted thoracic surgery upper lobectomy between March 2012 and November 2013. All operations were done by a single surgeon. From March 2012 to April 2013, division of the inferior pulmonary ligament was performed in all patients (N=43, right upper lobectomy: 27, left upper lobectomy: 16). From April 2013 to November 2013, inferior pulmonary ligament was preserved exclusively (N=29, right upper lobectomy: 11, left upper lobectomy: 18). In this study we retrospectively compared several postoperative variables.

Results : We found no significant difference in the duration of indwelling chest tube placement, duration of having chest tube drainage of more than 200ml, and pleural effusion in chest X-rays taken
one month after the operation in the two groups. There was no significant difference in the presence of
apical dead space between the two groups. There was also no significant difference in the change in
the bronchial angle between the two groups.

Conclusion : It is assumed that, in cases of upper lobectomy, whether inferior pulmonary ligament is divided or not, is not considerably related to postoperative complications.
