One-port One-window (OPOW) Technique in Thoracoscopic Surgery

Background : Since minimally invasive surgery has been developed, the number of ports has been reduced. In single-incision thoracoscopic surgery, there could be limitations with an exposure of surgical field and a working ergonomics to manipulate, which is potentially dangerous in hilar and node dissection. We evaluate the feasibility and effectiveness of one-port one-window (OPOW) technique.
Methods : From May 2012 to August 2014, seven patients underwent major pulmonary resection by OPOW at our institution. The medical records were retrospectively reviewed for the age, sex, surgical method, the median operative time, the mean length of chest tube drainage, the postoperative hospital stay and the mean pain score using Visual Analog Scale (VAS).
Results : The median age was 47 years old [range, 19-65 years] and male was 4. Six cases underwent lobectomy and one as wedge resection with mediastinal lymph node dissection (2 inflammatory lung diseases, 3 lung cancers, 2 bullous emphysema). Of six cases with lobectomy, one case underwent concurrent segmentectomy and wedge resection due to multiple metastatic nodules (Table 1). The median operative time was 190 minutes [range, 130-317 minutes]. The mean duration of drainage was 3.28±2.28 days and the mean hospital stay was 5.57±2.37 days. The mean VAS score was 5.71±2.29. There was no case requiring conversion to conventional VATS or thoracotomy and no occurrence of major perioperative morbidity and mortality.
Conclusion : OPOW technique showed better surgical field, comfort working ergonomics in hilar and node dissection, and comparable results with conventional VATS. It could be one of safe option in the surgical approach to pulmonary diseases.
