Efficacy of Pneumonotomy for Symptomatic Congenital Cystic Lung Disease in Neonates before Surgery

Background : Management of asymptomatic congenital cystic lung diseases remains controversial when addressing the optimal time of surgical resection. However symptomatic congenital cystic lung diseases in neonates should be resected because of life threatening emergency. Although emergency surgery is treatment of choice in neonates with symptomatic congenital cystic lung diseases, it may carry operative risks. The effect of pneumonotomy procedures in the emergent situation of congenital cystic lung diseases in neonates before surgery was analyzed.
Methods : The medical records of six neonates patients with congenital cystic lung diseases who underwent pneumonotomy procedure before surgery at Seoul Asan Medical Center between January 2011 and July 2014 were retrospectively reviewed. All the pneumonotomy procedures were performed pig-tail catheter by radiologist when there was respiratory distress or hemodynamic compromise in patients.
Results : There were no operative mortality and morbidity. 5 of the 6 neonates had diagnosed congenital pulmonary airway malformation(CPAM) (type 1; n=4, type 2; n=1) and 1 with pneumatocele. Their mean gestational age was 35 weeks, mean body weight was 2373g(range, 640g to 3790g). The most presenting emergent symptoms before pneumonotomy were respiratory distress and hemodynamic compromise for mediastinal shifting. After pneumonotomy procedure, all the patients had been relieved their symptoms, shortly and became hemodynamically stable. Mean time to interval of surgery was 18 days (range, 2days to 38days) and increased weight gaining was 363g (range, 80g to 1084g). All patients had undergone lobectomy with elective circumstance.
Conclusion : Pneumonotomy procedure is safe and beneficial in terms of relieving emergent respiratory and hemodynamic compromise in neonates with symptomatic lung disease surgery.
