Pulmonary Arteries Sutureless Patch Angioplasty Using Autologous Tissue Reconstruction Technique (“Pulmonary Arteries Peel Operation”) in Redo Congenital Cardiac Surgeries

Background : Reconstruction of branch pulmonary arteries (PAs) could be demanding in redo congenital cardiac surgeries. Treatment options including percutaneous balloon angioplasty and conventional surgical patch angioplasty may yield suboptimal results. PAs sutureless patch angioplasty using autologous tissue reconstruction (“PAs Peel operation”) can be a useful method to solve this problem.

Methods : We retrospectively reviewed 30 patients (19 males, and 11 females) who underwent PAs sutureless patch angioplasty between November 2004 and December 2013. The median age was 7.5 months (range: 4.8-96.4 months), and the median weight was 7.3 kg (range: 5.3-25.6 kg). RPA angioplasty was performed in 6, LPA angioplasty in 10, and both sides in 14 patients. Concomitant surgeries were bidirectional cavopulmonary shunt (BCPS) in 24, Fontan operation in 3, right ventricle-to-PA conduit change in 2 patients, and Rastelli operation in one patient.

Results : There was no operative death and two late deaths. All two late deaths were unrelated to PAs angioplasty. Mean follow-up duration after PAs angioplasty was 55.6 ± 28.9 months (range: 7.8-113.6 months). During follow-up, re-operation for PAs re-stenosis was required in 2 patients (6.7%), and balloon PAs angioplasty was performed in 4 patients (13.3%). Freedom from re-intervention at 1, 5, and 10 years were 96.7%, 83.9%, and 83.9%, respectively. Echocardiography or computed tomography (CT) angiography at the recent follow-up in most patients showed good branch PAs.

Conclusion : PAs sutureless patch angioplasty using autologous tissue reconstruction technique can simplify PAs angioplasty and be a safe & effective method for PAs reconstruction in redo congenital cardiac surgeries.
