The Renal Effect of Prophylactic Aminophylline Therapy after Cardiac Surgery in Infants

Background : Acute kidney injury (AKI) after pediatric cardiac surgery remains important complication. Aminophylline, an well established bronchodilator, is also considered to be an effective diuretic and anti-inflammatory agent. We evaluated the effect of prophylactic aminophylline treatment after cardiac surgery in infants.

Methods : From January 2009 to June 2014, 84 infants undergoing operation for isolated ventricular septal defect were classified to the prophylactic aminophylline therapy group (N=48) and the control group (N=36). This is a retrospective analysis of urine output, fluid overload and duration of oliguira/anuria between 2 groups in postoperative period.

Results : There was no statistical significance of pre- and intra-operative factors in 2 groups. Average daily urine output increased by 0.36 mL/kg/hr (p = 0.03), and daily input / output (I/O) balance decreased on average by 0.65 % per day in the postoperative 2nd and 3rd day (p = 0.01). There were no complications related to aminophylline administration.

Conclusion : We suggest that administration of aminophylline successfully promoted increased urine output in immediate postoperative period. The prospective studies are necessary to confirm the safety and effectiveness of prophylactic application.
