The Impact of SYNTAX Score on Graft Patency after Off-pump Coronary Artery Bypass Surgery (OPCAB)

Background : The aim of this study was to evaluate the impact of Syntax score on graft patency after off-pump coronary artery bypass surgery (OPCAB).

Methods : Of 1358 consecutive patients undergoing isolated OPCAB between January, 2008 and December, 2013. Of these patients, 228 of patients who had late computed tomography (CT) follow up were included in our study. The primary endpoint was at least one graft occlusion on the follow-up CT angiography. We compared syntax score between occlusion and patent group. The secondary endpoint was major adverse cardiac and cerebrovascular events (MACCEs).

Results : The mean interval from operation to late follow CT was 2.0 ± 1.2 years (range 0.6–5.0 years). Of the 740 anastomosis, 42 grafts (in 34 patients) were occluded (Overall graft patency: 94.3%). No significant difference was found in the preoperative SYNTAX scores between graft occluded 34 patients and the 194 patients without graft occlusion (mean syntax score 32.5 vs. 35.1, p = 0.16). In uni- and multivariate logistic regression methods, there was no correlation between graft occlusion and the SYNTAX score. There was no significant correlations between graft occlusion (p = 0.88) and MACCEs (p = 0.86) among patients with low (≤22), intermediate (23-32) and high (≥33)

Conclusion : The preoperative SYNTAX score might be not valuable as a predictor of graft patency and MACCEs.
