Long Term Outcomes of Aortic Root Replacement with a Prima Plus Stentless Valve for Aortitis Syndrome with Aortic Regurgitation – 3 Cases

Background : In the surgical management of aortic regurgitation with aortitis syndrome, prosthetic valve detachement is one of the serious complications. We report long term outcomes of a successful case of aortic root replacement with a Prima plus stentless valve (Edwards Lifesciences LLC, Irvine, CA, USA) for aortitis syndrome with aortic regurgitation.
Methods : Between January 2007 and December 2010, 3 patients with aortic regurgitation were treated with Prima plus stentless valve (Table 1). The ascending aorta completely transected about 10mm superior to the right coronary artery. Both coronary ostia were mobilized with buttons of aortic wall. The remaining tissue of the sinus of Valsalva and the disease aortic wall are excised. Prosthetics was attached to subvalvular left ventricular myocardium just below annulus, not annulus, with simple interrupted stitches of 2-0 polyester sutures. Then suture loops were tied down over about 5mm width strip of polytetrafluroethylene (PTFE) felt to support diameter of aortic annulus and to keep hemostasis of proximal suture line. Two openings were created in the prosthesis for implantation of the coronary ostia and coronary arteries were sewn to the prosthesis with continuous 5-0 polyprophylene sutures.
Results : The patients were finally diagnosed as Behcet’s disease. Each follow up duration was 84.4, 74.7 and 79.8 months. There is no structural valve deterioration or nonstructural dysfunction (Table 1).
Conclusion : Replacement of the aortic root and aortic valve with a Prima plus stentless valve is thought to be one of the surgical options for aortitis syndrome.
