The Predictor of Late Open Conversion Following Endovascular Abdominal Aortic Aneurysm Repair

Background : Endovascular aortic aneurysm repair (EVAR) is widely used for the treatment of abdominal aortic aneurysms. Complications secondary to EVAR are also treated with endovascular techniques. When this is not applicable, open surgical repair is mandatory. The present study aims to identify risk factors of late open conversion after endovascular abdominal aortic repair


Methods : A retrospective review was performed on prospectively collected data from all patients undergoing endovascular abdominal aortic repair from 1994 to 2014 at a single institution. Univariate analysis and multivariate Cox proportional hazards regression analysis were used to identify risk factors associated with late open conversion after endovascular abdominal aortic repair

Results : During the study period, 543 patients underwent at least 1 endovascular aortic repair. Of those, 24 patients underwent late open conversion. Mean time interval between EVAR and open conversion was 45 months (range, 2-190 months). The late open conversion rate was 4.4% (24 of 543). Hospital operative mortality was 8.3% (2/24). By multivariable Cox analysis, type I endoleak (adjusted hazard ratio 3.99, P=0.012), the use of additional stent-graft (adjusted hazard ratio 3.89, P=0.018), short neck <2cm) AAA (adjusted hazard ratio 6.12 p=0.021) were independent predictors of late conversion

Conclusion : EVAR is associated with continued risk of conversion, and surveillance may identify late complications that require removal, justifying lifelong monitoring. Aggressive monitoring for EVAR patients who have risk factors of late open conversion, such as type I endoleak, short neck (<2cm) AAA and the use of additional stent-graft is mandatory
