The Impact of Open and Endovascular Abdominal Aortic Aneurysm Repair on Renal Function

Background : Open (OR) and endovascular (EVAR) abdominal aortic aneurysm (AAA) repair can affect on renal function during the short and longer term and renal impairment is critical factor of morbidity and mortality. But which procedure is better on renal function is not clear. The goal of this study was to determine the impact of OR and EVAR on renal function during a period of 5years.

Methods : A retrospective cohort study was performed for 485 patients who underwent OR or EVAR for abdominal aortic aneurysm between February 2004 and December 2012. Of those, 379 patients who were available for creatinine level during 5 years follow up periods were enrolled in this study (mean age 68.6years, 89% men). The primary end points were change in estimated glomerular filteration rate (eGFR) calculated by the Chronic Kidney Disease Epidermiology Collaboration formula and conversion rate into ESRD at baseline 6months, 1year, 2years and 5years. Mean follow up duration was 2.85±1.78years.

Results : A total of 379 patients were included [OR=112 vs, EVAR=267]. Groups did not differ significantly in preoperative eGFR (71.2 vs 69.0 min/1.73 m2, p=0.439). On average, OR group lost a mean 10.70 min/1.73 m2 within 6month, 7.19 min/1.73 m2 after 1year and 8.22 min/1.73 m2 after 2years. The EVAR group lost a mean 4.88min/1.73 m2 within 6month, 6.10 min/1.73 m2 after 1year and 6.69 min/1.73 m2 after 2year. Finally 5 years after procedure, both group patients showed no significant declines of eGFR (6.95 vs 5.44 min/1.73 m2, P=0.68). Conversion rate into ESRD was also similar in both group (1.7% vs. 1.1%, P = 0.63).

Conclusion : Our study showed that there were no significant difference on impact on renal function between OR and EVAR group during 5 years follow-up periods. A further research with randomized design and long-term follow-up is needed to provide a definite answer.
