Cardiopulmonary Bypass Priming Using Autologous Cord Blood in Neonatal Congenital Cardiac Surgery

Background : This study aimed to assess the feasibility of cardiopulmonary bypass (CPB) priming using autologous cord blood in neonatal congenital cardiac surgery.

Methods : From September 2012 to August 2014, cord blood of fourteen patients had been collected during delivery and stored immediately in blood bank after informed consent. The six patients out of them used their own cord blood for CPB priming in neonatal congenital cardiac surgery.

Results : Screening serology and blood culture examination of cord blood revealed all negative. The congenital heart disease of six patients included total anomalous pulmonary venous return, transposition of great arteries with intact ventricular septum, coarctation of aorta with ventricular septal defect, hypoplastic left heart syndrome and interrupted aortic arch with tricuspid atresia. The median age and body weight at surgery was 9 days (from 1 to 21 days) and 3.21 kg (from 2.18 to 3.65 kg). The median amount and hematocrit of collected cord blood during delivery was 72.5 mL (from 43 to 100 mL) and 49.2 % (from 32.0 to 51.2 %). The median preoperative hematocrit of patients was 37.5 % (from 31.0 to 45.0 %). And the median amount of CPB priming was 130 mL (From 120 to 140 mL). The five out of six patients did not need allo-transfusion in CPB priming and only one patient used 20 mL of packed RBC in CPB priming to obtain target hematocrit.

Conclusion : The autologous cord blood can be used for CPB priming in neonatal congenital cardiac surgery. The auto-transfusion of cord blood can replace transfusion of packed RBC in CPB priming partially or totally.
