Single-port Video Assisted Thoracoscopic Surgery for Secondary Spontaneous Pneumothorax ; Comparison with Conventional Three-port Video Assisted Thoracic Surgery

Background : Single-port Video Assisted Thoracoscopic Surgery (VATS) is known to have better cosmetic effect, less incisional pain, and less paresthesia than the conventional three-port VATS. The aims of the present study were (1) to compare the postoperative results of single-port VATS and conventional three-port VATS in treatment of secondary spontaneous pneumothorax and (2) to elucidate advantages of the single-port VATS.

Methods : Thirty-four patients operated for the secondary spontaneous pneumothorax using VATS between July 2011 and December 2013 were included (single-port group, n=19 vs three-port group, n=15). Median duration of follow up was 124 days. The two groups were retrospectively compared in terms of operation time, postoperative complications, hospital stay, and pain score.

Results : There were no differences in preoperative characteristics, such as sex and previous pneumothorax episodes, between the 2 groups except age. The mean age was younger in single-port group (50.9 ± 17.9 years vs 66.8 ± 4.8 years, p = 0.004). There were no differences in visual analog pain scale on the day of surgery (4.53 ± 1.65 vs 4.63 ± 2.05, p = 0.299) or on postoperative day 1 (3.26 ± 0.99 vs 3.06 ± 1.16, p = 0.251) between the single-port and three-port groups. However, the single-port group showed shorter operative time (97.0 ± 34.9 min vs 123.9 ± 42.1 min, p = 0.014) and median hospital stay (2 [2,2] days vs 3 [2.5,5] days), p = 0.005). There were 3 complications (vocal fold palsy, wound infection, prolonged air leakage) in single-port group and 1 complication (prolonged air leakage) in three-port group (p = 0.613). During the follow-up period, there was no recurrent pneumothorax in both groups.

Conclusion : Single-port VATS proved to be a safe and feasible procedure with shorter operation time or length of hospital stay compared with the three-port VATS in secondary spontaneous pneumothorax patients. Progress in single-port instruments and surgical experience will lead to a wider application of single-port VATS.
