Surgical Treatment of Boerhaave’s Syndrome Using EEA Stapler

Background : Boerhaave’s syndrome is a fatal esophageal rupture that occurs during emesis. It has the worst prognosis of the esophageal perforation. From May 2007 to March 2014, 13 male patients underwent surgical treatment for Boerhaave’s syndrome using EEA(esophago-enteric anastomosis) stapler at Inje University Sanggye Paik Hospital
Methods : 13 patients diagnosed with the Boerhaave's syndrome underwent emergency surgery using EEA stapler in the Department of Thoracic and Cardiovascular Surgery of Inje University Sanggye Paik Hospital from May 2007 to March 2014. All the patients were male with a mean age of 51.4 years (range, 42-64 years).
Surgery was performed under one lung anesthesia, the left side lateral thoracotomy incision was made and the fifth or sixth intercostal space was appoached.
Results : Mean operation time was 142.5 minutes. One patient died because of mediastinitis and sepsis on post-operative day 37. There was no leakage for the other patients. Post-operative complications included one operative wound infection and one post-operative bleeding, which were treated completely.
Conclusion : Surgical treatment for Boerhaave’s syndrome using EEA stapler is a simple and effective technique. Further studies with a larger number of cases should be carried out for validation.
