Single Surgeon's Experience of Robot-assisted Mitral Valve Repair

Background : Mitral valve repair using Da Vinci surgical robot is well established clinical approach for isolated mitral regurgitation. The objective of this study is to review single surgeon's experience of robot-assisted mitral valve repair, compared with conventional open repair.

Methods : From March 2010 to February 2014, 17 patients underwent robot-assisted mitral valve repair (ROBOT group) using Da Vinci system (including concomitant tricuspid valve repair or maze procedure). We compared early and mid-term clinical results in this group with those from 21 patients who underwent conventional open repair (OPEN group, including minimally invasive thoracotomy approach) by single surgeon (C Lim) in the same period.

Results : Mean ages was 62.0±12.1 years and male vs female ratio was 20:18. There was no differences in preoperative demographics in both groups (table 1). There was no early mortality in each group. Major complication was not occurred. Even though cardiopulmonary bypass and aortic cross clamping times were significantly longer in ROBOT group (CPB: 194.8±48.6 vs 114.9±23.7 and ACC: 126.1±22.6 vs 75.1±16.8 minutes, both p<0.01), there were no difference in postoperative clinical results (table 2). Durations of postoperative mechanical ventilation and intensive care unit stay were shorter in ROBOT group. After 26.0±16.7 months of follow up, grade of mitral regurgitation decreased from 2.9±0.3 to 0.47±0.51. There is only one late mortality in OPEN group due to esophageal cancer.

Conclusion : Robot-assisted mitral valve repair showed excellent clinical results in mid-term, compared with conventional open repair, even though, long-term follow up will be mandatory.
