Risk of Cerebrovascular Events in Patients having Stroke History When Doing Off-pump Coronary Artery Bypass Grafting

Background : The incidence of stroke following coronary artery bypass grafting has gradually decreased in recent decades due to liberal use of off-pump technique and meticulous perioperative risk assessments. However, the impact of past stroke history on postoperative neurologic events in patients undergoing off-pump coronary artery bypass grafting (OPCAB) has sparsely reported.

Methods : We retrospectively reviewed 163 patients who underwent OPCAB between December 2012 and August 2014. 20 patients (12.3%) had past medical history of stroke (Stroke Group) and 143 patients (87.7%) had no stroke history (No-Stroke Group). Postoperative cerebrovascular events defined as a newly developed neurologic complications following operation, including cerebral infarction, cerebral hemorrhage, or transient ischemic attack.

Results : Postoperative cerebrovascular events occurred in 4 patients (2.5%). One patient excluded with transient ischemic attack on postoperative 5 months. Preoperative characteristics of patients were statistically similar in both groups. However, the Stroke group had more severe carotid artery stenosis (>70%) than No-Stroke group (6 /19 vs 10 /139; p=.001; Preoperative carotid CT angiography, N=158). Postoperative cerebrovascular events occurred in 2 patients in Stroke group while only in 1 patient in No-Stroke group (2/20=10% vs. 1/143= 0.7%; OR 15.78, p=.004).

Conclusion : The incidence of postoperative stroke in patients having stroke history was 10%. Patients with stroke history showed significantly higher risk of postoperative cerebrovascular events.
